Welcome to our Helpful Forms section, where you’ll find key documents to support your child’s day-to-day experience at school.

This includes our Walk Home Alone Permission Form, which is available only for children in Key Stage Two. If you wish to give consent for your child to walk home independently, please complete and return this form to the school office.

Please note:

  •  All children in EYFS and Key Stage One must be collected by a responsible adult aged 16 or over.
  • The adult collecting must also know your child’s password, which helps us ensure every child is safely handed over at the end of the day.

Administering Medicine Form – This must be completed for any medicine you wish the school to administer. Please note that prescribed medicine, such as antibiotics, will only be given if four doses are required during the school day. The completed form and medicine must be handed directly to the school office and must not be placed in school bags for safety reasons.

Holiday Request Form – If your child requires time off during the school term due to exceptional circumstances, this form must be completed and returned to the school office in advance. All requests will be reviewed in line with attendance guidelines.
